• You must complete the QuickPass on the day of your donation. Anything done earlier cannot be accepted.

• You must answer all of the questions honestly and in a confidential manner

• QuickPass does not determine your eligibility status to donate: This determination is made by qualified team members at the donation site

• If you are unable to access or complete your QuickPass, don’t worry, you can complete the questionnaire on site. 

• Completing the QuickPass is not the same as making an appointment, please click here to schedule your appointment.
